When is ACS Open?

ACS’s hours are by appointment Monday through Friday from 8 a.m. to 4:30 p.m. Please note that some of our clinics may have extended hours, and these will be posted in those clinics. We are closed on New Year’s Day, Martin Luther King, Jr.’s Birthday, Good Friday, Memorial Day, Fourth of July, Labor Day, Thanksgiving and the day after Thanksgiving, and Christmas Day.

How Do I Make an Appointment?

Please call 973-972-9000 to make an appointment.
If you are a new patient, please arrive 30 minutes before your appointment time. This will give you time for the registration process.

What Should I Bring?

  • a picture ID
  • your insurance card (if you have one)
  • proof of address, for example:
    • State issued ID
    • Current utility bill
    • Bank statement
    • Lease
    • Paystub
  • all of your medications including prescription, over-the-counter, vitamins, etc.
  • your prescription or referral from your primary care doctor or specialist, if appropriate
  • records, test results or x-rays you have from previous doctors that may help with your current condition.

How Long is the Average Visit?

Please allow about 2 hours on average for a visit. At ACS, we make every effort to get you in and out as quickly as possible. However, UH is a trauma center and major teaching hospital, and these factors sometimes add to the length of the visit, even in outpatient services. As a patient, your best results happen when you participate in your care. Please make every effort to attend every visit.

How Do I Cancel or Reschedule a Visit?

Please call 973-972-9000 to cancel and/or reschedule your appointment. Contact us as soon as possible if you are not able to keep your appointment so that we can release the time to other patients.

What If I Have Special Needs?

Please call 973-972-9000 in advance if you need a wheelchair when you arrive or if you need a translator or use of the deaf talk machine. There is no cost to you for these services.

When Do I Refill Medications?

To make sure that you don’t run out of medicine, contact the clinic when you have only a 5-day supply of your medication left. Please give the clinic 2 business days to process your request for refills.

How Will I Learn About Test Results?

A member of the healthcare team will contact you by phone or by mail if the results of any medical tests require additional follow-up. Please make sure you let us know if your phone number or address change. Your test results will also be available in MyChart.

What is MyChart?

MyChart is a new program that enables you to access portions of your health information online, including lab results, medications, appointment dates and immunizations. Ask any staff member how you can sign up, or click here.

Can You Help With Forms I Need to Complete?

We are happy to fill out insurance and medical clearance forms and referrals; turn-around is generally 5-7 business days.
