Open Public Records Act (OPRA) Requests

University Hospital, an instrumentality of the State of New Jersey, allows the public to request and review certain documents pursuant to New Jersey’s Open Public Records Act (N.J.S.A. 47:1A-1 et seq.). OPRA requests must be sent to the University Hospital Records Custodian via email at oprauh@uhnj.org, or via regular mail at University Hospital Records Custodian, Office of the Chief Legal Officer and Corporate Secretary, 150 Bergen Street, Newark, New Jersey 07103.

You may use the University Hospital OPRA Request Form included below to make your request. The form also explains your rights under OPRA and includes a portion that we will complete to respond to your request.
